Is it possible that a member could "mistakenly" blacklist another? Years ago, I seem to recall that it might have been a possibility but currently, admin included a prompt that asks if the member whether they are sure they want to continue with the blacklist or did they want to "cancel". My opinion, blacklisting doesn't happen by mistake.
